Step 1: Download and complete Registration Form.
Step 2: Mail completed registration form (mailing address on registration form) or email to partyforpoppak@gmail.com.
Step 3: Mail check to Peter Kareha. Please follow the directions on the form for writing checks; mailing address is also on registration form.
Unfortunately, we are not able to offer refunds for cancellations.
Step 4: You're done! Thank you for your support!
We are limited to the number of golfers we can have on the course, so please submit payment for golf by September 4, 2010 to ensure a spot. Everyone is invited to participate!
